Informed Consent for GHK-Cu Cream

INTRODUCTION

THIS INFORMED CONSENT (THE "CONSENT") FOR GHK-CU CREAM SETS FORTH THE TERMS AND POLICIES FOR THE CLINICAL SERVICES PROVIDED BY SUPERPOWER MEDICAL GROUP OF CA PC, A CALIFORNIA PROFESSIONAL MEDICAL CORPORATION, AND OTHER THIRD-PARTY MEDICAL GROUPS (COLLECTIVELY, THE "MEDICAL GROUPS") THROUGH THE ONLINE TECHNOLOGY PLATFORM ("PLATFORM" OR "SUPERPOWER PLATFORM"), WHICH IS OWNED AND OPERATED BY SUPERPOWER HEALTH, INC. ("SUPERPOWER").
The purpose of this consent is to provide you with information about the risks, benefits, and safety considerations associated with GHK-Cu Cream in order for you to make an informed decision as to whether or not to proceed with treatment, as well as to obtain your agreement to adhere to the treatment protocol as prescribed by your Medical Group healthcare provider ("Healthcare Provider"). Please read this form carefully and ask any questions you have before signing.

GENERAL INFORMATION

GHK-Cu (Glycyl-L-Histidyl-L-Lysine Copper) is a naturally occurring tripeptide with copper-binding properties that support skin repair, regeneration, and anti-aging effects. It has been widely studied for its role in promoting wound healing, improving skin elasticity, and reducing inflammation.
GHK-Cu is not FDA approved. The FDA states that compounded injectable drugs containing GHK-Cu may pose risk for immunogenicity due to the potential for aggregation and peptide-related impurities. There are limited data in humans to inform safety-related considerations. There is also limited data in humans to know of safety related considerations of GHK-Cu Cream.

How does GHK-Cu work (mechanism of action)?

  • Skin Repair: GHK-Cu promotes collagen and glycosaminoglycan synthesis, improving skin texture and elasticity.
  • Anti-Inflammatory Properties: It reduces oxidative stress and inflammation in the skin.

As with all therapies, there are potential benefits and risks.

Potential Benefits of GHK-Cu Cream:

TREATMENT PROTOCOL

Your Healthcare Provider will determine the appropriate concentration and frequency of GHK-Cu Cream based on your skin health, goals, and medical history.

Standard Usage Instructions:
  • Application Frequency: Apply a thin layer of GHK-Cu Cream to clean skin once or twice daily, as directed by your Healthcare Provider.
  • Application Sites: Typically used on the face, neck, or other areas requiring skin rejuvenation. Avoid mucous membranes and broken skin unless instructed otherwise.
  • Duration of Use: Treatment duration will vary depending on individual response and specific goals.
Monitoring Requirements:

Regular check-ins with your Healthcare Provider may be required to evaluate treatment progress and adjust dosage or frequency as needed.

Adherence:

It is critical to follow the prescribed application schedule and report any new or worsening skin reactions to your Healthcare Provider promptly.


ALTERNATIVES TO GHK-CU CREAM

There are alternative treatments for skin repair and anti-aging, including:

  • Topical treatments (e.g., retinoids, hyaluronic acid).
  • Non-invasive procedures (e.g., chemical peels, laser therapy).
  • Lifestyle modifications, such as improved diet and hydration.

These alternatives may not provide the same benefits as GHK-Cu Cream and should be discussed with your Healthcare Provider.

CONTRAINDICATIONS

You should not use GHK-Cu Cream if:

  • You have an allergy to any of its components.
  • You have a history of hypersensitivity to copper-based products.
  • You are pregnant, breastfeeding, or planning to become pregnant without consulting your Healthcare Provider.

ADDITIONAL SAFETY INFORMATION

  • If you experience severe side effects, such as a persistent rash or swelling, discontinue use and contact your Healthcare Provider immediately.

USE OF TELEHEALTH

Your Clinical Services will be provided via telehealth. Telehealth involves the use of electronic communication for diagnosis, treatment, and follow-up care. By agreeing to this Consent, you acknowledge the following:

  • Telehealth provides improved access to care but may limit the ability to perform a full physical examination.
  • There are potential risks, including delays due to technical issues or security breaches.
  • You must be physically located in the state where your Healthcare Provider is licensed during telehealth visits.

INFORMED CONSENT & ADHERENCE TO TREATMENT PLAN

By signing this informed consent, you acknowledge that you:

  • Have read and understood the information provided about GHK-Cu Cream, including its risks, benefits, and safety considerations. You understand that GHK-Cu Cream is not approved by the FDA to treat any disease or medical condition and the FDA states that compounded injectable drugs containing GHK-Cu may pose risk for immunogenicity due to the potential for aggregation and peptide-related impurities. There are limited data in humans to inform safety-related considerations of both injectable and compounded drugs containing GHK-Cu.
  • Agree to adhere to the treatment protocol as prescribed by your Healthcare Provider and report any severe side effects or concerns promptly.
  • Understand that there are no guarantees or assurances regarding treatment outcomes.
  • Certify that you have disclosed all relevant health information, including allergies, medications, and medical history to your Superpower Healthcare Providers.
